Inadequate bridge management was followed as Master steered the vessel out of a buoyed channel without asking for other’s recom- mendation. 4. Master ignored the VTS warning and acted following only his own opinion 5. Navigation Officer who should track the vessel’s position on paper chart every six minutes did not warn the Master for the shallow waters Lessons to be learned 6. Navigation plan needs to be carefully studied prior departure. 7. Bridge management is a team work and not “one man show”. 8. Only official equipment must be used (espe- cially Electronic charts as per IMO require- ments updated with the latest version of IHO). 9. Vessel position must be reported to Master (or OOW) in relation with nearby unsafe waters. 10. Leaving a buoyed channel is always dange- rous. 11.
